The overhead thrower frequently experiences shoulder pain as a result of anterior capsular laxity and increased demands placed on the dynamic stabilizers. This may be the results of repetitive high stresses imparted to the shoulder joint and may lead to the development of injuries. Most commonly, these injuries are overuse injuries and can be successfully managed with a well-structured rehabilitation program. The rehabilitation program should focus on the correction of adaptive changes seen in the overhead thrower, such a loss of internal rotation and muscular weakness of the external rotators and scapular muscles. The athlete may then initiate a gradual throwing program to return back to competition. The athlete’s overhead throwing motion should be examined to determine whether improper biomechanics are contributing to the injury.
Lastly, education of the athlete in the area of year-round conditioning is imperative. The throwing athlete should be instructed when to begin conditioning and throwing to prepare for the next competitive season and prevent subsequent injury.Current Concepts 2006 Shoulder Arthroscopy & Arthroplasty. The ambition of the Nice Shoulder Course, and therefore of this book, is to present the current state of knowledge and the latest concepts in both arthroscopic and arthroplastic shoulder surgery. There is no better way to find your way in the jungle than to follow “experimented guides”: these guides are world leaders in shoulder surgery who can make the difference between necessary and superfluous, between myths and facts.
I have been fortunate to assemble such a group of experimented guides, of whom many are close friends of mine. Their task has been to reveal to you the state of the art of shoulder arthroscopy and arthroplasty. All of them are international experts who have made major contributions to diagnosis, techniques, implant design and engineering principles in the shoulder. They all try to make shoulder pathology more understable and shoulder surgery more reproducible and more beneficial to patients. They have all accepted to share their knowledge without any restriction to help you to make the right diagnosis for your patients, to understand why certain reconstructive shoulder procedures should be performed and some others should not and, finally how those procedures should be performed in a more reliable way.

Proximal humerus fractures are generally the squeala of low energy falls in the elderly or high velocity trauma in younger patients and their incidence is increasing. Surgical management is dependent on the fracture pattern including the number of fragments and their emplacement, the associated lesions, bone quality and functional possibilities of the patient (i.e., their age). Many surgical approaches and procedures for fracture fixation are possible and include: closed reduction with percutaneous pin/screw fixation, open reduction and internal fixation with a locking or blade plate, intramedullary nailing, hemiarthroplasty and reverse shoulder prosthesis. Regardless of the type of fixation utilized, there are numerous pitfalls associated with the management of proximal humerus fractures that can results in poor functional outcome and the need for subsequent revision surgery.
Restoring shoulder anatomy and function after a proximal humeral fracture or fracture sequenlae is, without any contest, the most difficult challenge in the orthopaedics. The possibilities of creating iatrogenicity in treating proximal humerus fractures and fracture sequalea are almost unlimited… the main purpose of this book is, therefore, to try to at least avoid such iatrogenicity.
